- Provide RN assessment to determine skilled nursing needs
- Compile and submit PDN request form for Medicaid patients
- Compile letter of necessity and obtain physician signature
- Verify Medicaid and private insurance eligibility for services
- Coordinate discharge with equipment company, hospital and family
- Provide a nurse to accompany your patient home from the hospital
